Welcome to La Crosse School District Athletic Registration site. There are three steps you must follow to become fully registered for Athletic Participation at your school.
Physical Form (Click on link below and print if you need a new form...you just need to print the last page (PAGE 4) and turn that into your school's Activity Office. The form must be signed by a doctor/physician and a parent/guardian):
Parents and Student/Athletes are encouraged to complete this process together.
Step One: Read the Handouts below from the WIAA regarding Eligibility and Sportsmanship, the ImPACT Baseline Testing Consent Form (High School only), and the handouts about concussions (these links are below). There is a separate handout for student/athletes and parents. DO NOT print these forms! Just read them and complete the online registration process. By completing online registration, you will consent to the guidelines and expectations in each one of these forms.
Step Two: Complete the Registration Process by clicking the blue "View My Account" button located at the top, righthand side of this page. You will use your same log-in and password information as last school year (You should have created a log-in for THIS registration website. This process is not the same as Skyward registration). If this is your first year registering, you will need to create a log-in.
**If you create a log in, it will be used for future school years and your information will be saved...Use your email as your log in name so it's easier to remember**
Step Three: Submit your signed Physical Form (if needed). This can be scanned in via the online registration process, or turned into your school's athletic director.
**Sports physicals dated April 1, 2021 or after are good for the 2022/23 school year. Physical forms must be signed by a doctor and parent/guardian.**
